Job summary

BJC Behavioral Health is seeking a Community Outreach Case Worker to connect clients with housing and food resources, provide home visits, and support recovery in the community. The role requires daily field work, a valid driver’s license, and the ability to travel locally.

Training is provided during the first 90 days to ensure proficiency with our programs. This full-time position offers a sign-on bonus up to $1,500 and opportunities across the North, Central, and South territories.

Qualifications

• Bachelor's Degree in Human Services or related field.
• No prior experience required.

Responsibilities

• Provide community support services to complex clients per care plan with treatment interventions.
• Complete timely documentation detailing service need, intervention, dates, and response.
• Contact individuals or referrals after missed appointments to re-engage and support recovery.
• Offer mentorship and job shadowing to community support colleagues.
