Zusammenfassung

BJC Behavioral Health is seeking a Community Outreach Case Worker to connect clients with housing and food resources, provide face-to-face community support, and travel locally to client homes. This full-time role includes a sign-on bonus up to $1500, daily community work, and a Monday–Friday day schedule with no nights or weekends.

Training is provided in the first 90 days. Qualified candidates will need a Bachelor’s in Human Services or related field, with a valid Class D (IL) or Class E (MO)

Qualifikationen

• Bachelor's Degree in Human Services or related field is required.
• No prior experience required.
• Class D (IL) or Class E (MO) license required or to be obtained prior to start date.

• Provides community support services to clients to maximize recovery and community integration.
• Completes timely documentation detailing services, interventions, and client responses.
• Contacts clients and referral sources after missed appointments to re-engage and support recovery.
• Provides mentorship and job shadowing to colleagues in community support.
