Job summary

Akido Labs, Inc. in Long Beach is seeking a Community Health Worker to support unhoused patients with complex medical and social needs, educating, coordinating care, and promoting self-management in the community.

The role involves managing a caseload, building rapport, and guiding patients to appropriate services while partnering with clinical teams. Travel is heavy, and fluency in English and Spanish is highly preferred.

Qualifications

• High School Diploma or equivalent.
• 2+ years of experience with CalAIM populations (homeless, SMI/SUD, post-incarceration, complex medical needs).
• Travel 75% required; valid driver’s license and reliable transportation.
• Bilingual in English and Spanish strongly preferred.

Responsibilities

• Manage a caseload of patients and develop care plans with patients and clinical teams.
• Build rapport to encourage positive behavior changes.
• Coach patients to minimize risks related to health conditions and behaviors.
• Accompany members to medical and social services appointments.
• Connect members to programs to address barriers to care and enhance compliance.
• Link members to local, county, and state services; advocate for members.
