Job summary

Akido Labs in Long Beach, CA is seeking a Community Health Worker to support members with complex medical and social needs, helping them manage their health and wellbeing through education, peer support, and shared experiences.

This role involves managing a caseload, developing care plans with patients and clinical teams, accompanying members to appointments, connecting to local services, and promoting self-management in an outdoors-focused environment.

Qualifications

• High School Diploma or equivalent.
• 2+ years’ experience with CalAIM populations (homeless, SMI/SUD, post-incarceration, complex medical needs).
• Willingness to travel around 75% and valid driver’s license.
• Bilingual in English and Spanish preferred.
• Ability to work outdoors and independently as well as in a team.
• Strong communication and documentation skills.
• Proficient in documenting in case management software.

Responsibilities

• Manage a caseload of patients and develop care plans with patients and clinical teams.
• Build rapport to encourage positive behavior changes.
• Coach patients to minimize risks from identified conditions.
• Accompany members to medical and social services appointments.
• Connect members to programs and services to address barriers.
• Link members to local, county and state services and follow up.
• Document encounters in designated systems.
• Outreach and enroll eligible patients into the Akido ECM Program.
• Other duties as assigned.
