Job summary

Akido Labs, Inc. in Los Angeles seeks a Community Health Worker to support members with complex medical and social needs, focusing on CalAIM populations. The role includes in-person outreach, case management, education, and linking patients to local services and programs.

You will manage a caseload, build rapport, accompany members to appointments, document encounters, and promote self-management in an outdoor, dynamic environment. 75% travel, bilingual English/Spanish preferred.

Qualifications

• High School Diploma or equivalent required.
• 2+ years experience with CalAIM populations or related social/health work.
• Valid driver’s license and ability to travel ~75%.
• Bilingual in English and Spanish preferred.

Responsibilities

• Manage a caseload of patients, complete assessment forms, and develop care plans with patients and clinical teams.
• Build rapport with patients to encourage positive behavior changes.
• Coach patients to minimize risks related to health conditions and behaviors.
• Accompany members to medical and social services appointments.
• Connect members to local, county, and state services and advocate for them.
• Document information from encounters in designated systems.
