Job summary

Qualifications

• 2+ years of experience with CalAIM populations of focus (homeless, adults with SMI/SUD, post-incarceration, adults with complex medical needs).
• Must have a valid driver's license, automobile insurance and reliable transportation; travel up to 75%.
• Bilingual in English and Spanish strongly preferred.

Responsibilities

• Manage a caseload of patients and complete assessment forms to develop care plans with patients and clinical teams.
• Build rapport to encourage positive behavior changes and coach patients to reduce health risks.
• accompany members to medical and social services appointments and connect to relevant programs.
• Document encounters in designated case management information systems.
• Engage eligible patients in the Akido ECM Program and perform outreach.
• Collaborate with local, county and state services; advocate for members.
• Promote self-management and self-efficacy among members.
