Job summary

Cityblock is hiring for a care coordination role in Montgomery County, MD. You will work closely with RN Care Managers to support members with complex social, behavioral, and physical needs, visiting in home or community settings and coordinating care with providers.

Qualifications

• 2+ years of progressive experience in care coordination or related field.
• Bachelor's degree required.
• Unrestricted driver's license and daily vehicle use.
• Proficiency with scheduling platforms and electronic health records.

Responsibilities

• Collaborate with RN Care Manager to develop and adjust member care plans.
• Visit members at home or in community settings to support care coordination.
• Provide non-clinical education on preventative care topics to members.
• Coordinate with internal and external providers to connect members with services.
• Maintain timely documentation of member interactions and outcomes.
