Job summary

Cityblock Health is seeking a Care Coordinator to support members with complex medical and behavioral health needs. You will manage a diverse caseload, develop member-centered care plans, and collaborate with RN Care Managers to address social determinants of health.

The role involves assessments (PHQ-9, GAD-7, AUDIT, DAST-10) and referrals to behavioral health programs, with strong emphasis on community-based care and partner collaboration to close quality gaps.

Qualifications

• 2+ years of progressive experience in care coordination, community health, health promotion, or related field.
• Strong problem-solving skills with good judgment and ability to escalate clinical concerns.
• High-quality written, verbal and presentation communication; experience with documentation.

Responsibilities

• Collaborate with RN Care Managers to support members with complex medical and behavioral health needs.
• Manage a caseload of members, identifying and addressing social, behavioral, and environmental barriers to health.
• Develop and implement care plans with members, addressing social needs and care goals.
• Conduct assessments including PHQ-9, GAD-7, AUDIT, DAST-10 and refer to behavioral health programs as needed.
• Support members in closing quality gaps by assisting with provider appointments, lab tests, and referrals.
• Meet members in community settings (home, shelter, hospital) as needed as an extension of the care team.
• Document interactions, track tasks, and communicate with the care team and community partners using scheduling platforms.
