Job summary

Amity Foundation invites compassionate individuals to join our ECM CHW team. The role focuses on care coordination, improving health outcomes, and reducing unnecessary healthcare utilization for students.

You will bridge ECM students with medical and community resources, with field-based duties and local travel required. Required: degree in social work/psychology/public health or related field (or high school + 3 yrs outreach), plus 2+ yrs in client-facing work.

Qualifications

• Bachelor’s degree or equal experience in outreach or care services.
• Minimum two years of client-related experience in case management or outreach.
• Equivalent of a high school diploma with three years in outreach and reentry services.
• Strong communication and interpersonal skills; ability to engage diverse populations.
• Willingness to travel locally to work with students in the community.

Responsibilities

• Coordinate care for ECM students and act as liaison with healthcare providers.
• Engage ECM students with barriers to care and support appointment attendance.
• Assist RN Care Manager and BH Care Manager in panel management and referrals.
• Support outreach, case reviews, and collaboration with the ECM team.
• Accompany students to office visits and assist with resource navigation.
• Distribute health promotion materials and connect to social services.
• Advocate for students with providers and monitor treatment adherence.
