Careforth, Inc. in Raleigh, NC is seeking a Community Engagement Specialist to promote our Structured Family Caregiving program.

You will cultivate partnerships with local care coordinators, AAAs, and other organizations, while visiting families at home to conduct family-centered assessments. You will collaborate with a centralized coaching team to support caregivers, document assessments, and coordinate coaching interventions.

Formación

• Bachelor’s degree in social work or equivalent experience.
• Minimum two years of case management experience.
• Experience with elders or people with disabilities preferred.
• Strong communication and problem-solving skills.
• Knowledge of home and community services.

Responsabilidades

• Promote Structured Family Caregiving and generate referrals.
• Build partnerships with local organizations.
• Conduct home visits and collect assessments.
• Collaborate with coaching team and care coordinators.
• Assist caregivers with credentialing and app setup.
• Travel within assigned geographic area.
