Optum CA is seeking a Field Based Clinical Practice Performance Coordinator in Chatsworth, CA

Position Overview

This role involves driving clinical relationships and engagement with physician practices and members, collaborating with internal teams such as Network Contract Managers, Analytics, Medical Directors, Supportive Care Programs, and Market Leaders. Responsibilities include improving practice performance, managing chronic condition documentation, HEDIS, and STARs gap closure, providing education and reporting, and identifying performance improvement opportunities through data analytics and workflow enhancements.

The position is full-time, Monday to Friday, with flexibility to work any 8-hour shift between 8:30 AM and 5:00 PM PST. Occasional overtime may be required. The office is located at 9301 Oakdale Avenue, Chatsworth, CA 91311. Training will be onsite from 8:30 AM to 5:00 PM PST, Monday to Friday.

Primary Responsibilities
  • Coordinate high-volume office workflows and programs with Practice Relationship Managers (PRMs).
  • Conduct office assessments and develop supportive action plans.
  • Maintain and update office action plans in collaboration with stakeholders.
  • Host meetings and engagement sessions with offices and IPA staff.
  • Provide training, system support, and education on tools like Cozeva and Datacore.
  • Address system challenges and escalate as needed.
  • Coordinate education sessions with medical directors.
  • Support quality and risk documentation through chart reviews.
  • Enter supplemental data and manage regional event planning for clinics and health fairs.
  • Assist with workflows for high-volume offices and support PRMs with pursuit list generation.
Qualifications

Required:

  • High school diploma or GED or equivalent work experience
  • Must be 18 years or older
  • At least 2 years of healthcare experience
  • Ability to drive up to 75% of the time and reliable transportation
  • Flexibility to work full-time, Monday to Friday, during business hours, with occasional overtime

Preferred:

  • Experience with Windows programs (Word, Excel, PowerPoint, Outlook)
  • Medical Assistant experience in a physician's office
  • Knowledge of HEDIS and Risk Adjustment
  • Fluency in Spanish, Cantonese, Vietnamese, or Mandarin
Compensation and Benefits
