Positions in this function focus on clinical relationships and engagement with physician practices and members, partnering internally with teams such as Network Contract Managers, Analytics, Medical Directors, Supportive Care Programs, and Health Plan leaders, aiming to improve practice performance. Responsibilities include initiatives like chronic condition documentation, HEDIS, and STARs gap closure, supporting network offices through education and reporting, and proactively identifying performance improvements via data analytics, technology, workflow, and clinical support. These roles develop provider-specific plans to enhance physician practice performance, reduce readmissions, and improve outcomes.

This position is full-time, Monday - Friday, with flexible 8-hour shifts during business hours (8:30 am - 5:00 pm PST). Occasional overtime may be required. The office is located at 9301 Oakdale Avenue, Chatsworth, CA 91311.

Training is provided on-site, with hours from 8:30 am - 5:00 pm PST, Monday - Friday.

Primary Responsibilities:
  1. Advance high-volume office workflow and program coordination with PRMs.
  2. Conduct office assessments and develop supportive action plans.
  3. Maintain office action plans in collaboration with stakeholders.
  4. Host regular meetings and engagement sessions with offices and IPA staff.
  5. Provide training, system support, and education on tools like Cozeva and Datacore.
  6. Liaise on system challenges and escalate issues appropriately.
  7. Coordinate education sessions with medical directors.
  8. Support quality and risk documentation through chart dives.
  9. Manage quarterly event planning for clinics and health fairs.
  10. Support workflows for high-volume offices.
  11. Collaborate with PRMs on pursuit list generation and reinforce messages in the IPA office.
Required Qualifications:
  • High school diploma/GED or equivalent work experience
  • Must be 18 years or older
  • At least 2 years of healthcare experience
  • Ability to drive to local offices up to 75% of the time
  • Reliable transportation
  • Flexibility to work full-time, Monday - Friday, during business hours with possible overtime
Preferred Qualifications:
  • Experience with Windows programs (Word, Excel, PowerPoint, Outlook)
  • Medical Assistant experience in a physician’s office
  • Knowledge of Quality (HEDIS) and Risk Adjustment
  • Fluency in Spanish, Cantonese, Vietnamese, or Mandarin

The salary range is $41,300 to $80,800 annually, based on full-time employment and factors such as experience and location. Benefits include a comprehensive package, incentive programs, stock purchase options, and 401k contributions.
