Job summary

Responsibilities

• Build relationships with internal teams and partner providers.
• Manage an active caseload and track progress monthly.
• Prepare reports and update service plans per regulations.
• Lead PCSP process and assist with HCBS services.
• Coordinate LTSS and Medical Assistance access for members.
• Ensure HIPAA compliance and confidentiality.
• Travel ~75% of time; field-based work.
