Job summary

Qualifications

• Bachelor's degree in social work, psychology, or related field with practicum experience preferred OR at least 3 years in a social service or healthcare setting.
• Strong case management and care coordination background.
• Experience conducting in-home assessments and visits.
• Ability to work with adults with complex medical, behavioral, and social needs.

Responsibilities

• Establish strong relationships with team members and providers to coordinate resources.
• Manage an active caseload with regulatory reporting and compliant timelines.
• Prepare reports and update service plans; ensure timely comprehensive assessments.
• Lead person-centered service planning and help Members access LTSS and HCBS services.
• Coordinate LTSS/Medicaid services and other supports; assist with fair hearing requests when needed.
• Maintain HIPAA compliance and confidentiality; work 75% in the field and remotely as needed.

Education

Bachelor's degree in social work / psychology / related field
