Job summary

Humana Inc. is seeking a dedicated Case Manager to serve Wisconsin Family Care members in Milwaukee County and surrounding areas. You will assess needs, develop a comprehensive care plan, and coordinate with nursing staff to deliver services in the least restrictive environment.

This role requires a valid driver’s license and strong documentation discipline. The position includes travel up to 40% and standard M-F hours (8:00 am–4:30 pm CST).

Qualifications

• Bachelor's degree in human services or related field is required; 1 year of experience serving frail elders or adults with disabilities.
• Alternatively, a Bachelor's degree in another field with at least 3 years of such experience is also acceptable.
• Valid state driver’s license and proof of personal vehicle liability insurance with at least 100/300/100 limits.

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Conduct quarterly visits and monthly phone contact with members.
• Arrange non-medical supports such as housing, employment, and community resources.
• Maintain accurate documentation including case notes and MCP updates.
