Job summary

Humana Inc. is seeking a dedicated Case Manager for the Wisconsin Family Care program within Inclusa/Humana. You will assess needs, develop comprehensive plans, coordinate with nurses, and conduct regular in-person and phone check-ins with frail elders and adults with disabilities.

The role requires a Bachelor's degree in human services or related field with 1+ year of relevant experience or 3+ years in another field, plus a valid driver's license and insurance. Travel up to 40% is expected.

Qualifications

• Bachelor's degree in human services or related field required.
• At least 1 year of experience serving frail elders or adults with IDD/physical disabilities; or 3 years in another field with relevant experience.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Conduct quarterly in-person visits and monthly phone contact with members.
• Arrange non-medical support services (social integration, housing, employment, community resources).
• Ensure cost-effective service delivery and maintain documentation (MCP, authorizations).
• Educate members by evaluating risk factors.
