Job summary

Humana is seeking a Case Manager to support Wisconsin Family Care members in La Crosse County and surrounding areas. You will perform comprehensive case management, coordinate with a Field Care Nurse, and conduct regular visits and assessments in members’ homes to ensure safe, effective care.

Responsibilities include developing a Member Care Plan, arranging non-medical services, and maintaining accurate MCP documentation.

Qualifications

• Bachelor’s degree in human services or a related field with at least 1 year of experience, or a Bachelor’s degree in another field with at least 3 years of relevant experience.
• Valid state driver’s license and proof of personal vehicle liability insurance with at least 100/300/100 limits.

Responsibilities

• Assess members health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Conduct quarterly in-person visits and maintain monthly contact by phone.
• Arrange non-medical support services including social integration, community resources, housing, and employment.
• Ensure cost-effective service delivery.
• Evaluate risk factors and provide education to members.
• Maintain documentation including case notes, service authorizations, and MCP updates.
