Job summary

Molina Healthcare is seeking a Care Coordinator in Illinois to support care management for members with high-need potential. You will perform comprehensive assessments, develop and monitor waiver service plans, and collaborate with an interdisciplinary team to coordinate services across LTSS.

The role requires strong communication, organization, and the ability to travel locally up to 25-40%. You will educate members, caregivers, and providers using Molina clinical guideposts to promote quality

Qualifications

• At least 2 years of health care experience including LTSS exposure.
• Experience in care management or medical/behavioral health setting.
• Valid driver's license and reliable transportation for travel.
• Ability to work independently and manage multiple priorities.
• Strong verbal and written communication skills.
• Proficiency in navigating online portals and databases.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care teams.
• Monitor care plans and adjust interventions as needed.
• Promote integration of LTSS with behavioral health and community resources.
• Assess medical necessity and authorize services.
• Collaborate with interdisciplinary care team for approvals.
