FreedomCare in Massachusetts is seeking a Care Manager to lead in-home assessments, determine eligibility for Home Health services, and coach caregivers to deliver high-quality, compassionate care in patients' homes around Boston.

The role combines hands-on field visits with ongoing support, coordinating with the care team, clinicians, and leadership to ensure safety, compliance, and positive outcomes. A relevant degree and MA licensure or equivalent clinical experience are required.

Qualifications

• Bachelor’s degree and MA social worker license or clinical experience with elders or disabilities.
• Massachusetts tuberculosis screening negative.
• 2+ years of health coaching, care coordination, patient support, or community health experience.
• Valid driver’s license and reliable transportation.
• Strong empathy, communication, and teamwork.

Responsibilities

• Conduct in-home assessments for initial and ongoing evaluation of caregiver quality of care.
• Provide coaching and education to caregivers via in-person and LMS trainings.
• Support consumers with service questions and next steps within non-clinical scope.
• Document coaching interactions, outcomes, risks, referrals, and follow-ups.
• Develop and revise client care plans in collaboration with clinicians.
• Coordinate care and escalate concerns to clinical and leadership teams.
