Job summary

You will act as the primary contact, support transitions of care, reinforce care plans, and coordinate with primary care, specialists, pharmacies, and community providers.

Responsibilities include conducting structured interviews, performing home visits, addressing social needs, and delivering culturally competent education.

Qualifications

• 3+ years of direct patient care in ambulatory/primary or senior care.
• Experience discussing chronic conditions and reinforcing medication instructions.
• Ability to conduct home visits and community outreach.

• Perform home visits to observe living conditions and barriers to engagement.
