Job summary

Community HealthWorks is seeking a Community Health Worker to support Medi-Cal members in the Sacramento area as part of CalAIM. The role emphasizes field engagement, care coordination, and linkage to essential services in homes, shelters, clinics, and community sites.

You will conduct assessments, coordinate benefits, and help members navigate housing and health coverage while maintaining accurate documentation and collaborating with health plans and partners.

Qualifications

• High school diploma or GED required.
• Ability to build respectful relationships with people from varied backgrounds.
• Clear communication, organization, confidentiality, and follow-through.

Responsibilities

• Conduct in-person and telephone outreach to Medi-Cal members identified for Enhanced Care Management or Community Supports.
• Build trust, help members identify goals, understand options, and take next steps.
• Meet members in community settings and accompany them to appointments when appropriate.
• Help members obtain food, housing, and essential resources.
• Document services and communicate with teams and health plans.
