Job summary

Creating A Legacy Inc. is seeking a Behavioral Health Supervisor to oversee services for adults with developmental disabilities in home and community settings.

You will provide hands-on field supervision, coach staff, monitor quality, and ensure implementation of individualized plans with a focus on consumer safety, dignity, and independence. Responsibilities include regular home visits, staff coaching, incident follow-up, and collaboration with clinical and administrative teams.

Qualifications

• High school diploma or equivalent required.
• At least two years of experience supporting individuals with developmental disabilities, behavioral support needs, or mental health needs.
• At least one year of supervisory, lead staff, or comparable leadership experience.
• Experience following individualized service plans and behavior support plans.
• Current CPR and First Aid certification, or ability to obtain within onboarding period.

Responsibilities

• Provide field supervision and oversight of services to adults in homes and community settings.
• Coach direct support staff and monitor service delivery against plans.
• Conduct regular home visits, observations, and coaching with staff.
• Coordinate with Scheduling to ensure appropriate coverage and manage scheduling changes.
• Review incident reports and ensure timely follow-up and documentation.
• Support consumer safety, dignity, and choice through person-centered approaches.
