Job summary

UnitedHealth Group is seeking a Behavioral Health Advocate to support Medicaid enrollees in Western Suffolk County. This field-based role requires travel to facilities and homes, with 80% onsite meetings and 20% remote work.

A NY license in LMHC/LMFT/LCSW/LP and 2+ years of mental health experience are required. Responsibilities include coordinating community services, aftercare, and liaising with providers to develop aftercare plans.

Qualifications

• Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage and Family Therapy, or a related field.
• Active, unrestricted NY license as LMHC, LMFT, LCSW, or LP.
• 2+ years of experience in a related mental health environment.
• Proficient in Microsoft Office including Word, Outlook, Excel, and Teams.
• Valid driver's license and reliable transportation for daily travel.
• Dedicated home office with high-speed internet.

Responsibilities

• Identify and provide community service linkage.
• Assist enrollees with aftercare appointments and follow-ups.
• Coordinate care with providers and treatment teams to prevent readmission.
• Travel to hospitals, clinics, and community sites as needed.

Education
