Job summary

Qualifications

• Requires a Master's degree in Behavioral Health or Social Work or a Nursing degree with BH experience.
• Experience in behavioral health care settings and with complex care management is preferred.
• Excellent communication, assessment, and documentation skills required.

Responsibilities

• Evaluates member needs via phone or in-home visits and develops care/service plans.
• Coordinates with family, caregivers, and providers to ensure timely access to services.
• Provides education to members and families on procedures, options, and benefits.
• Monitors outcomes and updates care plans based on changes in condition or needs.
