Job summary

Optum is seeking a Behavioral Health Advocate to provide field-based case management and care coordination for adult Medicaid members across Manhattan. The role focuses on supporting behavioral health needs, reducing hospitalizations, and coordinating transitions of care with community resources.

The BHA serves as a liaison between health plan teams, providers, treatment facilities, and community organizations, aiming to reduce readmissions and improve community stability.

Qualifications

• Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage & Family Therapy, or related behavioral health field.
• Active New York independent license (LCSW, LMHC, LMFT, or Licensed Psychologist).
• 2+ years of post-licensure experience in a mental health setting.
• Proficiency with Microsoft Office (Word, Outlook, Excel, and Teams).
• Residence in or near Manhattan with reliable transportation for community-based visits.

Responsibilities

• Provide field-based behavioral health case management for adult Medicaid members in Manhattan.
• Coordinate aftercare planning and transition-of-care services.
• Connect members with behavioral health, medical, and community resources.
• Partner with hospitals, treatment teams, and community providers.
