Competenze

• Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage and Family Therapy, or related field.
• Active, unrestricted NY license as LMHC/LMFT/LCSW/LP.
• 2+ years in a mental health setting.
• Proficient with Word, Outlook, Excel and Teams.
• Dedicated home workspace with secure, high‑speed internet.
• Valid driver’s license and transportation for daily travel in Buffalo area.

Mansioni

• Identify and provide community service linkage.
• Assist enrollees with aftercare appointments.
• Provide follow-up per enrollee needs.
• Identify gaps in care and problem-solve access to services.
• Review IP Census daily to flag re-admits in past year.
• Attend facility meetings with treatment teams to plan aftercare.
• Manage aftercare follow-up for high‑risk enrollees.
• Meet with providers to foster relationships and identify programs.
