Job summary

HealthFirst seeks an Assessment Nurse to perform field and virtual assessments for members in downstate New York, determining enrollment and eligibility using the Uniform Assessment System. You will coordinate with the Care Management Team to support long-term care plans such as CompleteCare and Senior Health Partners.

You will travel across downstate NY, participate in care planning, and liaise with families and home care agencies to ensure accurate service needs and timely documentation.

Qualifications

• New York State RN license is required.
• Must be able to travel across downstate New York including NYC boroughs, Long Island, Rockland and Westchester.
• PEDS position requires pediatric clinical field experience or related work with child-serving systems.

Responsibilities

• Conducts home assessments (and virtual assessments when needed) for prospective and current members using the Uniform Assessment System (UAS).
• Determines enrollment and continued eligibility through clinical judgment.
• Reviews medical notes to update Care Plans or assist with nursing home admissions.
• Performs in-person Care Management visits to support person-centered service planning.
• Liaisons between members and the Care Management Team; assesses home environment and psychosocial status.
• Provides feedback to home care agencies and the CMT regarding issues and performance.
• Delivers community-based training for frail populations on self-care and prevention.
