Job summary

Healthfirst is seeking an Assessment Nurse to conduct field assessments for members in downstate New York. This role involves evaluating service needs and determining eligibility for managed long-term care plans while working closely with the Care Management Team.

The ideal candidate holds a New York State RN license, can travel throughout New York City, and is capable of assessing members' care needs effectively. Knowledge of health insurance and experience with frail populations are desirable.

Qualifications

• New York State RN license required.
• Ability to travel around downstate New York including the 5 boroughs.
• Experience with pediatric or medically fragile populations is preferred.

Responsibilities

• Conduct home assessments for prospective and current members.
• Review previous assessments and determine necessary adjustments.
• Act as a liaison between the member and Care Management Team.
