Job summary

UnitedHealth Group in Greenwich, CT is hiring a PRN RN Field Assessor to perform Long Term Care insurance assessments. You will collect demographics, physician information, current medications, and evaluate functional independence to determine care needs.

This role requires tech-savvy use of a point-of-care web application on a laptop or tablet for real-time documentation. A valid RN license and CPR are required, plus a valid driver’s license and access to reliable transportation.

Qualifications

• Current and unrestricted RN licensure in the state.
• Current CPR certification.
• Ability to function in any home setting regardless of client background.
• Valid driver’s license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Perform Long Term Care insurance assessments.
• Obtain claimant demographics and physician information.
• Evaluate claimant's functional independence for ADL/IADL.
• Complete cognitive screening and document findings.
• Use a point-of-care system on a laptop or tablet for real-time documentation.
