Qualifications

• Must hold New York State RN license.
• Ability to travel around downstate NY incl. 5 boroughs, Long Island, Rockland, Westchester.
• Experience navigating multiple tech platforms and modalities.
• Proficient in Word, Excel, and Outlook.
• Strong time management, communication and problem-solving skills.

Responsibilities

• Conduct home and virtual assessments for prospective and current members.
• Navigate UAS software, EMR systems, TruCare to document assessments.
• Determine service needs and adjustments through Care Plan reviews.
• Perform in-person Care Management visits and liaise between members and CMT.
• Provide community-based training and ensure member enrollment in LTCP.
