Healthfirst in New York City seeks an UAS Nurse Assessor to conduct home and virtual assessments for prospective and current members, using the Uniform Assessment System to determine enrollment and ongoing eligibility.

You will navigate UAS software and electronic medical records, perform person-centered Care Management visits, and liaise with the Care Management Team to update care plans, with a flexible 4x10 schedule and strong benefits.

Formación

• Licensed Registered Nurse with active license.
• Experience performing nursing home or home assessments preferred.
• Proficient with UAS, EMR systems and related tools.

Responsabilidades

• Conducts home and virtual assessments using the Uniform Assessment System (UAS).
• Determines enrollment eligibility and service needs for members and families.
• Uses UAS software and EMR to document assessments accurately.
• Reviews nursing notes to update Care Plans and support Care Management decisions.
• Performs in-person Care Management visits and liaises with the Care Management Team.
