Position Summary
The Family and Community Well-Being (FCW) Specialist - Home Visits supports the implementation of the Cultivate Wellness program by conducting scheduled home visits, providing approved gardening and wellness education, documenting participant engagement, and connecting households with appropriate community resources.
Cultivate Wellness is a free, at-home gardening and health program that empowers families to grow fresh, culturally relevant produce while increasing access to affordable, healthy food. Through hands-on gardening education and regular home visits, staff support participating households with garden development, health education, service referrals, and personalized guidance that promotes healthier lifestyles and long-term well-being.
The Specialist works directly with an assigned group of households and serves as a consistent point of contact throughout the growing season. The position follows established lesson plans, visit schedules, safety procedures, and documentation requirements while communicating participant questions, concerns, and follow-up needs to the FCW Coordinator or Manager.
This is a temporary, part-time seasonal position requiring approximately 15-20 hours per week during the Cultivate Wellness program season, generally from late April through August, with the possibility of continued employment through September based on program needs and available funding.

Employment Period: Generally late April through August. Employment may be extended through September to support post-program surveys, participant follow-up, evaluation, and other program activities, depending on program needs and available funding.
Benefits Eligibility: This temporary position is not eligible for LCM's employer-sponsored medical, dental, or vision insurance, paid holidays, paid time off (PTO), or other benefits reserved for eligible regular employees, except as required by applicable law or governing benefit-plan terms.
Compensation: Employees will be paid for all hours worked in accordance with applicable wage-and-hour requirements, including overtime when applicable.
Temporary Employment: Employment is limited to the seasonal assignment and does not guarantee continued employment, renewal, conversion to a regular position, or a minimum number of weekly hours.

Essential Duties and Responsibilities
Home Visit Coordination and Participant Support
Manage an assigned caseload of Cultivate Wellness households based on program needs, household locations, and travel distances.
Contact assigned families to schedule, confirm, and, when necessary, reschedule home visits in coordination with the FCW Coordinator.
Conduct routine home visits using approved lesson plans, checklists, educational materials, and program procedures.
Provide participants with timely and accurate program information and communicate schedule changes, workshops, events, and other opportunities.
Develop consistent, respectful relationships with participating households to support engagement and continuity throughout the season.
Identify participant questions, barriers, and follow-up needs and communicate them promptly to the FCW Coordinator.
Conduct initial visits and visits involving sensitive or complex concerns with the FCW Coordinator or FCW Manager, as directed.
Maintain appropriate professional boundaries and follow established home-visit safety and communication procedures.
Gardening and Wellness Education
Provide basic gardening education and demonstrations related to planting, watering, soil care, staking, pest management, harvesting, and seasonal garden maintenance.
Observe garden progress and help families address routine gardening questions using approved program guidance and resources.
Deliver approved wellness education activities in the intended sequence and avoid combining multiple lessons into one visit unless directed by the FCW Manager.
Deliver plants, tools, supplies, educational materials, and other approved program resources to participating households.
Encourage family participation, goal setting, and the use of culturally relevant produce in healthy meals.
Connect households with appropriate community resources that address identified needs.
Provide approved educational information without offering medical advice or services outside the position's training and responsibilities.
Data and Documentation
Document attendance, home-visit activities, educational topics, participant questions, referrals, and follow-up needs accurately and promptly in Apricot or other approved systems.
Administer approved pre- and post-program surveys, assessments, and participant feedback tools according to established program procedures.
Maintain accurate and current records for assigned households and submit required documentation by established deadlines.
Collect participant reflections, testimonials, and program observations when requested to support program evaluation and continuous improvement.
Protect participant information and follow all confidentiality, recordkeeping, and data-security requirements.
Safety, Communication, and Professional Conduct
Follow LCM policies related to home-visit safety, confidentiality, professional boundaries, mandatory reporting when applicable, and incident reporting.
Notify the FCW Manager promptly of health, safety, housing, or social-service concerns that require professional follow-up.
Communicate schedule disruptions, missed visits, participant concerns, and urgent needs to the FCW Coordinator in a timely manner.
Follow established safety procedures when entering participant homes, working outdoors, transporting supplies, and traveling between locations.
Use LCM identification, phones, supplies, and other program resources responsibly and return all assigned property at the conclusion of employment.
Represent LCM professionally in participant homes, at community locations, and in all written, telephone, and electronic communications.
Program and Team Support
Participate in required orientation, training, team meetings, and case-review meetings.
Coordinate with the FCW Coordinator and other program staff to support consistent service delivery and timely participant follow-up.
Assist with Cultivate Wellness workshops, garden builds, plant and supply distribution, outreach, and seasonal events as assigned.
Share participant feedback and field observations that may improve program materials, scheduling, education, or services.
Assist with post-program surveys, documentation, participant follow-up, and seasonal program closeout activities as assigned.
Other
Complete approved local travel between participant homes, LCM sites, and community locations.
Accurately record and submit all hours worked in accordance with LCM timekeeping procedures.
Support special projects and perform other duties related to the position as assigned by the FCW Coordinator or FCW Manager.

Qualifications
Successful completion of the Siembra Salud internship, prior Cultivate Wellness experience, or comparable LCM or community-based program experience preferred.
Experience working directly with families or supporting community-based health, wellness, gardening, education, or social-service programs preferred.
Demonstrated reliability, professionalism, respect for participants, and ability to work independently within established procedures.
Bilingual proficiency in Spanish and English required.
Ability to complete required documentation accurately and promptly.
Ability to work approximately 15-20 hours per week during the primary program season, generally from late April through August, with the possibility of continued work through September to complete post-surveys and program follow-up.
Availability for weekday morning and afternoon home visits and occasional Friday or Saturday workshops or program activities, as assigned.
Successful completion of required background checks, orientation, and LCM training.
Certificates, Licenses, and Registrations
A valid driver’s license, reliable transportation, and proof of auto insurance are required. First Aid/CPR/AED certification may be required and may be obtained through the employer after hiring. No other certificates, licenses, or registrations are required.
