Job summary

La Casa de las Madres is seeking a Case Manager – FCS Specialist in San Francisco, CA. You will provide comprehensive services to families involved with CPS or referred to HSA/FCS, including crisis intervention, safety planning, counseling, and home visits.

You will conduct risk assessments, coordinate with community resources, and support families through multi-site needs while maintaining confidential documentation and collaborating with PSW and staff.

Qualifications

• BA/BS in Behavioral Sciences preferred; 5+ years of verifiable case management experience.
• Experience working with CPS/public agencies and community organizations.
• Bilingual English/Spanish preferred; strong written and verbal communication.

Responsibilities

• Provide comprehensive services to families experiencing domestic violence, including crisis intervention, safety planning, counseling, case management, home visits, and parenting education.
• Conduct risk/lethality assessments and organize skill-building and peer support sessions.
• Manage referrals to community resources and coordinate with FCS and PSW teams.
• Maintain confidential client files and complete program documentation in line with grant requirements.
• Present to staff and clients about La Casa's services and build community partnerships.
