POSITION SUMMARY

The Fatherhood Case Manager is an experienced human services professional responsible for providing comprehensive case management, participant support, and administrative coordination for the Fatherhood Program. Working with a high degree of independence, this position helps fathers strengthen family relationships, increase engagement with their children, overcome barriers to stability, and achieve personal and family goals.

The Fatherhood Case Manager collaborates with participants, service providers, and community partners to connect fathers with employment, educational, health, and social service resources. Responsibilities include participant recruitment and outreach, facilitation of parenting and fatherhood education groups, maintenance of program documentation, data entry, and progress reporting.

The ideal candidate is committed to father engagement, family strengthening, and community-based service delivery and uses a strengths-based approach to support positive outcomes for fathers, children, and families.

GENERAL DUTIES/ RESPONSIBILITIES:

Case Management and Participant Support

• Establish and maintain positive, professional, and supportive relationships with program participants.
• Conduct assessments to identify participant strengths, needs, goals, and barriers.
• Assist participants in achieving goals related to child support, paternity, visitation, custody, and co-parenting.
• Connect participants with employment, educational opportunities, and community resources that support self-sufficiency and family stability.
• Assist participants in accessing health, behavioral health, housing, and other supportive services as needed.
• Maintain an active caseload and ensure regular contact with participants to monitor progress, evaluate needs, and promote engagement.
• Promote positive parenting practices, healthy relationships, and responsible father involvement.
• Model effective engagement strategies and encourage growth through a strengths-based and reflective practice approach.

Group Facilitation and Education

• Facilitate fatherhood education groups utilizing the 24/7 Dad curriculum.
• Incorporate financial literacy and financial stability concepts into group sessions.
• Promote healthy lifestyles and substance abuse prevention strategies.
• Foster a supportive and inclusive group environment that encourages participation, learning, and personal development.
• Monitor participant engagement and document attendance, progress, and outcomes.

Outreach, Recruitment, Marketing, and Community Engagement

• Promote Fatherhood Program services to parents, caregivers, referral sources, and community stakeholders.
• Participate in local and statewide initiatives that support fatherhood and family strengthening efforts.
• Assist in the planning, implementation, and coordination of program events and initiatives.
• Develop and maintain collaborative relationships with community organizations and referral partners.
• Support participant recruitment efforts through community outreach, presentations, marketing activities, and networking.

Administrative and Operational Responsibilities

• Complete required documentation, including assessments, service plans, case notes, referrals, and reports in a timely and accurate manner.
• Maintain participant records in accordance with agency policies, funding requirements, and confidentiality standards.
• Enter and manage program data within designated databases and tracking systems.
• Collaborate with agency staff and program leadership to ensure enrollment goals, service targets, and program outcomes are achieved.
• Participate in staff meetings, training opportunities, and professional development activities.
• Engage in quarterly file reviews and quality assurance activities to maintain compliance and service excellence.
• Perform other duties as assigned by the Program Supervisor, Director, or agency leadership.

PQI Functions

• Ensure the consistent and accurate collection of data.
• Identify patterns and trends within the program.
• Use data results to inform the supervisor of trends that may impact services.

QUALIFICATIONS:

Education and Experience

• Associate degree preferred; equivalent combination of education and relevant experience may be considered.
• Experience in social services, case management, family support services, workforce development, or a related field preferred.
• Experience working with fathers, families, underserved populations, or community-based programs is highly desirable.

Knowledge, Skills, and Abilities

• Strong verbal, written, and interpersonal communication skills.
• Proficiency in Microsoft Office applications, including Word, Excel, and Outlook.
• Ability to establish rapport and maintain effective working relationships with diverse populations.
• Knowledge of community resources, family support services, and human service systems.
• Demonstrated cultural competence and commitment to providing equitable and inclusive services.
