Qualifications

• Associate’s degree in behavioral science or related field, or equivalent work experience.
• Ability to learn case management principles and reimbursement requirements.
• Excellent organizational and time management skills.
• Effective verbal and written communication skills.
• Ability to relate to and interact with a wide variety of people.

Responsibilities

• Coordinate and provide care that is client-centered and safe.
• Maintain caseload and regular client contact to monitor progress.
• Draft service plans and review case progress for closure with supervisor.
• Provide ongoing services including crisis intervention, advocacy, budgeting, housing assistance, and referrals.
• Develop individualized treatment plans and ensure policy compliance.
• Document clinical activities to meet program and funder criteria.
• Participate in treatment team and support status change recommendations.
• Provide client education and facilitate group curriculum per program requirements.
