Job summary

Talbert-House in Cincinnati, OH is seeking a Case Manager for the Fatherhood Program. This full-time professional role focuses on individualized coaching, goal setting, and connecting fathers with community resources within Brown County.

You will manage caseloads, develop service plans with supervision, and deliver supportive counseling while documenting activities to meet program and funder requirements.

Qualifications

• Basic computer skills in Word, Outlook, Excel, Teams, PowerPoint

Responsibilities

• Coordinate and provide care under minimal supervision to be safe, timely, effective, efficient, equitable and client-centered
• Maintain caseload and monitor progress through regular client contact
• Draft service plans and review progress with supervisor for closure decisions
• Deliver ongoing services including crisis intervention, advocacy, medication monitoring, transportation assistance, budgeting, housing assistance and referrals
• Support wellness and autonomy through counseling and care coordination
• Develop and implement individualized client treatment or service plans and ensure compliance with standards
• Document clinical activities timely to meet program, funder and credentialing criteria
• Participate in treatment teams and advise on client status changes
• Provide client education and facilitate group curriculum per program requirements
• Adhere to professional standards and complete assigned duties
• Potentially provide prevention education in school/community programs (up to 30%)
