Job summary

Hope For Youth, Inc. in Bay Shore, NY seeks a full-time Youth Peer Advocate or Family Peer Advocate to support children and families in the ACT program. The role emphasizes family engagement, community-based supports, and collaboration with clinicians.

You will work within policy guidelines, participate in trainings, and provide transportation as needed to ensure consistent client access to services and services integration.

Qualifications

• Credential or willingness to obtain the Family Peer Advocate Credential within a set timeframe.
• Lived experience as a parent or caregiver to a child with mental health diagnosis.
• Proficiency with Microsoft Office Suite; experience with EHR systems.

Responsibilities

• Function within program policies and procedures.
• Work within an interdisciplinary team to assist client functioning and outcomes.
• Provide rehabilitative support services to families and caregivers.
• Help families develop community connections and natural supports.
• Teach and model strategies to support child behavior; conduct parent skill development.
• Attend trainings and daily/team meetings.
• Apply Evidence Based Interventions to meet client needs.
• Provide transportation for clients to service locations as needed.
• Perform other duties to meet program and regulatory requirements.

Hope For Youth's (HFY) Youth Assertive Community Treatment (ACT) is a program licensed by the NY State Office of Mental Health and designed to address the significant needs of children ages 10 up to 21, who are at risk of entering, or are returning home from high intensity services, such as inpatient settings or residential services, using a multi-disciplinary team. Children with significant psychiatric needs, who are at risk of institutional level of care, require intensive interventions to adequately support the child and family's complex needs, to avoid high end services or facilitate and support a successful transition back to community.

Our team has an opening for a full-time Youth Peer Advocate (YPA) or a Family Peer Advocate (FPA) in the ACT program.

To be a successful Family Peer Advocate you must be ready, willing, and able to do the following always:

1. Function within the policies and procedures as outlined in program practice guide and employee manual
2. Adhere to state regulations and applicable code of conduct
3. Work within an interdisciplinary team to assist client in their overall functioning and achieve desired outcomes
4. Provide rehabilitative support services through an array of formal and non-formal services to the parent, family member or caregiver for the benefit of the child/youth.
5. Assist families with developing community connections and naturals supports to enhance the quality of life by integration and supports for families in their own communities, as well as helping the family to rediscover and reconnect to natural supports already present in their lives
6. Helping the family learn and practice strategies to support their child's positive behavior, assisting the family to implement strategies by clinicians, providing individual or group parent skill development related to the behavioral health needs of the child (i.e., training on special needs parenting skills).
7. Attend trainings and professional development as assigned
8. Attend daily team meetings and family meetings as assigned
9. Apply agency-identified Evidence Based Interventions best suited to meet the client's needs
10. Provide transportation for client to their service location as necessary
11. Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations

How you will generally spend your time in this job:

• 60% Direct family therapeutic contact including transportation
• 20% Case documentation, reports, case related meetings
• 20% Staff/team meetings/supervision/training

Minimum Qualifications to have and maintain for this role are:

1. Family Peer Advocate Credential or willingness to obtain the credential within a set timeframe
2. Lived experience as a parent or caregiver to a child with mental health diagnosis
3. Computer proficiency, including Microsoft Office Suite, and experience working with EHRs
4. Highschool Diploma or GED
5. Valid NYS driver's license in good standing
6. Physical Requirements: read/write reports and log entries, walking/standing/sitting for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, daily driving own vehicle (mileage reimbursement available).
