Job summary

Integrated Services for Behavioral Health (ISBH) in Pickaway County seeks a Care Coordinator to support children, youth, and families through home, school, and community-based coordination. You will identify needs, connect with community resources, and ensure services align with safety and treatment goals.

This role emphasizes teamwork with Child and Family Teams, cultural humility, and cross-systems collaboration.

Qualifications

• Experience providing services to children and families connected to behavioral health.
• Hi-Fidelity Wraparound knowledge preferred (Certification provided at hire).
• Experience with autism spectrum disorders and developmental disabilities preferred.
• Be culturally humble or responsive with training to manage complex cases.
• Excellent organizational skills and ability to manage multiple tasks.

Responsibilities

• Identify care coordination needs with families to ensure best outcomes for children and youth.
• Define cultural factors influencing strengths to maintain engagement and care planning.
• Coordinate family-based services in home, school, and community.
• Ensure services meet safety and treatment needs and are least restrictive.
• Engage with child and family teams to integrate care plans.
• Develop partnerships with community resources to meet diverse needs.
