Job summary

Seven Hills Community Services is seeking a Family Liaison- Tech Navigator to empower families by connecting them with community resources and teaching them how to use smartphones, tablets and apps for independence and health management.

You will work with a multidisciplinary team, deliver coaching, and provide assistive technology training to families and participants in home and community settings.

Qualifications

• High school diploma or GED required.
• Standard driver’s license – class C or D; some roles may not require a license.
• CPR/First Aid certification required.
• 1–3 years of related experience.

Responsibilities

• Foster connections between families/participants and community services.
• Advocate for the multidisciplinary team on behalf of the family and participant needs.
• Equip parents with tools to manage behavioral needs and health navigation.
• Provide informal coaching and cultural mediation.
• Develop and monitor holistic, person-centered care plans.
• Provide health and safety education and resources to families.
• Participate in 24-hour caregiver/participant consultation on a rotating basis.
• Maintain confidential health records and complete required documentation.
