Responsibilities

• Full-time Family Support Worker needed to assist families in addressing challenges and navigating services, providing emotional support, and helping them build skills and resilience to improve their lives.
• Assessing Needs: Conduct assessments to identify the needs and goals of families.
• Case Management: Develop support plans and monitor progress through regular follow-up.
• Connecting Resources: Link families to community resources, advocate, and support access to services.
• Crisis Intervention: Provide crisis support as needed.
• Support Groups: Facilitate support groups and provide education on relevant topics.
• Home Visits: Conduct early developmental screenings, offer guidance on prenatal and maternal health, and parenting skills for pregnant women and parents of young children.
• Documentation: Maintain accurate case records.

Qualifications

• Education: Bachelor’s or master’s degree in social work or related field.
