Job summary

Urban Strategies, Inc. is seeking a Family Support Specialist II in New Bern, NC to provide comprehensive case management and coordinate with community services under HUD’s Choice Neighborhoods grant.

You will work with social workers, case managers, and relocation/employment staff to support families and seniors. Responsibilities include intake/assessments, developing care plans, data entry in LEARN, and attending meetings.

Qualifications

• Bachelor’s degree in social work, sociology, psychology or related field is required.
• Minimum of three years of experience as a case manager or in direct practice with low-income households.
• Ability to handle privileged information confidentially and work well in a team.
• Proficiency in Microsoft Outlook, Word and Excel is required.

Responsibilities

• Conduct intake and assessments for residents into the case management program.
• Conduct home and office visits to collect information from residents.
• Develop and monitor Development Plans with a multidisciplinary team.
• Navigate local social services and public agencies for residents.
• Identify gaps in services and advocate on behalf of residents.
• Enter and track resident data in LEARN and document progress in real time.
• Attend community meetings and participate in project-related activities.
• Be available for occasional nights, weekends, and travel.
• Perform administrative tasks including reports and billing documentation.
