Qualifications

• Bachelor's degree in Social Work, Psychology, or related field preferred.
• One year of experience providing evidence-based home visiting services preferred.
• Must have an F endorsement and be able to drive for work-related duties.

Responsibilities

• Initiates and maintains regular long-term contact and support with families for five years with each family.
• Ensures services are provided in the family's home.
• Each visit lasts about one hour and uses family-centered and strengths-based approaches.
• Maintains case management milestones and Healthy Families America standards.
