Family Support Specialist

Position Summary

We are seeking a compassionate, organized, and dedicated Family Support Specialist to provide direct support and advocacy services to children, families, and caregivers. The Family Support Specialist will work collaboratively with community partners, service providers, and internal teams to assess family needs, develop service plans, connect families to resources, and promote family stability and well-being. The ideal candidate will have strong communication skills, experience working with diverse populations, and a commitment to empowering families through a strengths-based approach.

Essential Duties and Responsibilities
  • Conduct assessments to identify family strengths, needs, and barriers to success.
  • Develop and implement individualized family service plans.
  • Provide case management, advocacy, and ongoing support services.
  • Connect families with community resources, including housing, healthcare, employment, education, and behavioral health services.
  • Maintain accurate and timely case documentation and reports.
  • Monitor family progress and adjust service plans as needed.
  • Facilitate educational workshops, support groups, and family meetings.
  • Collaborate with schools, healthcare providers, social service agencies, and community organizations.
  • Assist families in crisis situations and provide appropriate referrals.
  • Ensure compliance with agency policies, funding requirements, and confidentiality standards.
Minimum Qualifications
Required Education
  • Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Family Studies, Counseling, or a related field.
Required Experience
  • Minimum of one (1) year of experience working with children, families, or vulnerable populations preferred.
  • Experience in case management, family support services, community outreach, or social services is highly desirable.
Knowledge, Skills, and Abilities
  • Knowledge of family systems, child development, and community resources.
  • Strong interpersonal, communication, and problem-solving skills.
  • Ability to build trusting relationships with families from diverse backgrounds.
  • Excellent organizational and time-management skills.
  • Ability to maintain accurate records and documentation.
  • Proficiency with Microsoft Office and case management software.
  • Ability to work independently and as part of a multidisciplinary team.
Preferred Qualifications
  • Bilingual proficiency (Spanish and English) preferred.
  • Experience working with at-risk families, child welfare, early childhood programs, or behavioral health services.
  • Certification in case management or family support services is a plus.
Physical Requirements
  • Ability to travel within the service area to conduct home visits and attend community meetings.
  • Ability to sit, stand, and use a computer for extended periods.
  • Valid driver's license and reliable transportation may be required.
