BCFS Health and Human Services seeks a Family Support Specialist to provide case management and crisis intervention for at‑risk youth and families. You will work with diverse populations, coordinate services, and maintain compliance with state and federal standards, often traveling to client homes and partner sites.

Qualified candidates will have a bachelor’s degree in social work or related field, or four years of direct service experience, plus strong communication and bilingual (Spanish)

Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice or four years' direct service experience with at-risk youth and families.
• Experience providing direct services to vulnerable populations.
• Demonstrated ability to work with diverse populations in a fast-paced environment.

Responsabilités

• Provide individual and family sessions using Motivational Interviewing and Strengthening Families Program.
• Coordinate referrals, service planning, and documentation for assigned caseload.
• Respond to crises promptly and professionally during office visits, home visits, or calls.
• Develop, monitor, and review action plans to increase strengths and address needs at home.
• Assist in emergency shelter placements and/or respite care when appropriate.
• Maintain confidentiality and follow safety protocols.
• Attend staff meetings, trainings, and supervision sessions.
• Travel to client homes and community partners as needed.
• Meet deadlines with program supervisor and state/federal partners.
