Job summary

Mercedes Diaz Homes Inc is seeking a Coordinated Family Support Instructor - BIL English/Mandarin pref in Whittier, CA. This full-time role supports individuals with intellectual and developmental disabilities in their homes and community through daily routines and activities.

Qualifications include a High School diploma or GED, an Associate degree in human services or 3 years of relevant experience, a valid driver's license, and English proficiency. Bilingual abilities are preferred.

Qualifications

• High School diploma or GED or equivalent; be at least 18 years old.
• Associate degree in human services or 3 years of relevant experience with ID/DD, behavioral challenges, dual diagnoses, or forensic involvement.
• A valid driver’s license and CA automobile insurance to drive to community locations, independently or with team members.
• Ability to read, write and speak English; bilingual in English/Spanish, Vietnamese, Cantonese, or Mandarin preferred.

Responsibilities

• Provide support to Individuals with intellectual/developmental disabilities in daily routines and recreational activities.
• Coordinate with families and circle of support to ensure safe, healthy home environments and progress toward Individual Service Plan goals.
• Document and report accurately and timely; maintain proactive communication with stakeholders.
