Job summary

The University of Miami/UHealth Department of Pediatrics is seeking a full-time Case Worker 1 in Miami, FL. You will help patients keep appointments, identify barriers to care, develop treatment plans, and monitor delivery of services ordered by health care providers.

Responsibilities include evaluating patients to inform plans, enrolling in insurance and social services, and supporting enrollment in ACA exchanges to maintain continuity of care.

Qualifications

• Bachelor’s Degree required from an accredited university with emphasis in psychology, social work, health education, interdisciplinary sociology, early childhood, child development or special education
• One year of relevant experience with young children and families in teaching, case management or counseling, or with individuals with special needs
• Knowledge of various data systems and ability to learn within databases
• Ability to analyze, organize and prioritize work under pressure with deadlines
• Ability to handle confidential information with discretion
• Ability to work evenings, nights, and weekends as necessary

• Ability to work independently and/or in a collaborative environment

Responsibilities

• Conducts patient evaluations to develop effective treatment plans
• Enrolls patients in insurance and social service programs
• Learns ACA enrollment processes for care continuity
• Identifies and resolves barriers to care
• Assists with patient-provider communication
• Provides medication adherence education and counseling
• Conducts secondary prevention and lifestyle management interventions
• Educates families on community resources and programs
• Maintains confidentiality per HIPAA and regulations
• Documents services and supports record audits
• Coordinators with interdisciplinary team for IFSP and services
