Job summary

Communitas, Inc. in Wakefield, MA is seeking a Family Support Coordinator to provide home-based services for families with DDS needs. You will translate family needs into available community supports, and assist with access to SSI, CommonHealth, and behavioral supports under DDS guidance.

Your role includes coordinating with DDS and DCF, developing individualized treatment plans, and documenting progress for meetings, with a focus on empowering families and enhancing outcomes.

Qualifications

• Bachelor's Degree in Social Work, Education, Human Services or a related field is required.
• Up to two years of family support case management experience preferred.
• A passion and dedication to supporting our people is a must.
• Valid Driver’s license, reliable transportation, and acceptable driving record.
• Must pass company background and reference checks.

Responsibilities

• Inform the family and assist them in accessing supports and services within the community.
• Assist families in obtaining state and federal entitlement benefits such as SSI, CommonHealth, and behavioral supports.
• Understand Massachusetts Special Education Laws and work with collaterals and other agencies.
• Attend intake and review meetings at DDS and develop Individualized Family Treatment Plans.
