Job summary

Communitas, Inc. in Wakefield, MA is seeking a Family Support Coordinator to assist families with children eligible for DDS services.

The role offers full-time, 30 hours per week at $24 per hour, with home-based services to identify needs and connect families with services and community supports. The position requires a Bachelor’s degree in social work or related field and up to two years of family support case management experience; a valid driver’s license and ability to pass background checks

Qualifications

• Bachelor's degree required.
• Up to two years of family support case management experience preferred.
• Must pass company background and reference checks.

Responsibilities

• Inform the family and assist them in accessing supports and services that exist within the community.
• Assist families in obtaining all state and federal entitlement benefits such as SSI, CommonHealth clinical/medical supports, and behavioral supports.
• Have an understanding of the Massachusetts Special Education Laws.
• Assist families in working with collaterals.
• Collaborate with other agencies.
• Supervise support staff working in families’ homes.
• Attend all Intake and Review Meetings at DCF.
• Develop and Implement Individualized Family Treatment Plans.
• Document progress and prepare reports for DCF review meetings.
• Complete discharge summaries for all families and all necessary documents on I Family Net.
• Identify services and teach skills to aid parents in keeping, or returning, their children home.
• Have knowledge of child development.
• Sensitivity to poverty and cultural issues.

Education

Bachelor's Degree in Social Work, Education, Human Services or a related field
