Job summary

UCP of Western Massachusetts in Easthampton, MA is seeking a Part-Time Family Support Advocate to assist adults with disabilities in the home and community. You will provide direct 1:1 support, help with daily living skills, and guide members in activities and interests to promote independence.

The role requires time in the community and documentation, with collaboration from families and clinical teams to ensure consistent care.

Qualifications

• High School Diploma or equivalent required.
• One year of human service work or six months in a related field with life experience with individuals with disabilities.
• Trauma-informed care training preferred.
• Proficiency with computers for case notes, email, and video conferencing.

Responsibilities

• Provide direct, 1:1 support in the home and community.
• Assist with daily living activities and promote independence.
• Implement safety and behavioral support strategies as trained.
• Maintain professional boundaries and accurate documentation.
• Collaborate with the family and clinical team to ensure consistent care.
