Job summary

CN Guidance and Counseling Services, Inc. seeks a qualified professional to join the FACT Team, delivering services to individuals with severe mental illness whose needs are unmet by traditional approaches.

The role includes assessments, cross-system coordination, and a mix of direct and telehealth services in office or community settings. The position requires a bachelor’s degree, New York license, and experience in integrated mental health and substance abuse treatment, with a 35-hour workweek

Qualifications

  • Bachelor's degree or higher in a related field.
  • Two years’ experience with chronically mentally ill recommended; master’s level preferred.
  • NY Driver's License, clean DMV, and proof of insurance required.
  • Competency in substance abuse, family support, or vocational rehabilitation demonstrated.

Responsibilities

  • Perform routine team duties and lead integration of dual-recovery and support with other team members.
  • Provide assessment and service planning, QA processes, utilization review, and inter-system coordination.
  • Engage in assessments, case management, crisis intervention, and discharge planning.
  • Maintain case records and complete documentation on time in compliance with regulations.
  • Provide direct, telehealth, face-to-face, and/or collateral services to recipients in office or community settings.
  • Work a 35-hour week with pager coverage as arranged.

Job Definition

Work with the CN Guidance FACT Team to deliver services to individuals diagnosed with severe mental illness whose needs have not been met by traditional approaches.

A bachelor's degree or higher in social work, psychology, rehabilitation counseling, or a related field is preferred.

Experience Required

Two years' experience working with the chronically mentally ill (master's level) recommended, or three years' experience at a bachelor's level. Competency in substance abuse, family support, or vocational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment.

Duties and Responsibilities
  1. Perform routine team duties and lead the integration of dual-recovery treatment, family support, or vocational/employment training with the tasks of other team members.
  2. Provide assessment and service planning, quality assurance and improvement processes, utilization review, incident reporting, and coordinate between FACT and other systems.
  3. Engage in assessments, case management, crisis intervention, and discharge planning.
  4. Maintain case records and complete documentation on time in compliance with federal, state and agency regulations.
  5. Provide direct, telehealth, face‑to‑face, and/or collateral services to recipients in the FACT office or in the community where problems arise.
  6. Work a 35‑hour week, including periodic pager coverage as arranged with the Team Leader or Assistant Team Leader.
  7. Assist recipients in developing expectations from treatment.
  8. Strengthen the recipient’s ability to understand and utilize therapy and find resources after treatment through proper discharge planning and termination processes.
  9. Exchange information with other agencies to benefit the recipient, ensuring compliance with regulations and coordination of services.
  10. Utilize and understand the value of supervision, which increases practice skills, promotes better services, and preserves accountability.
  11. Facilitate sharing of data with individuals to enable partnered decision‑making and keep clients informed of progress.
  12. Complete QI training during onboarding and participate in QI refresher training as needed.
  13. Cultivate a culture of inclusion for all employees that respects individual strengths, views, and experiences.
  14. Adhere to all safety protocols and procedures to ensure a safe working environment for all employees.
  15. Demonstrate health and safety awareness to maintain a safe work environment.
  16. Perform other duties as assigned.
